There are many factors which may play a role in the development of PCOS, for example it has a very strong genetic link, so a person may be more prone to developing it if they have a close relative who has it, it is strongly linked to obesity due to the higher risk of insulin resistance, chronic stress can play a major role due to the effect of cortisol on sex hormone balance and lack of exercise and poor nutrient status may also contribute.
